New Clients
To become a Skinlab client, Oklahoma state law requires you to complete a Virtual Health Assessment with our Physician Assistant before receiving any injectable or laser treatments.
- Create your Skinlab account on our Boulevard Portal.
- Choose your location — Norman or Nichols Hills.
- Click “Individual Appointment.”
- Select “New Client? Click here.”
- Complete your telehealth appointment with our provider.
- Once that’s done, you can book your desired treatment anytime!
Clients of Shelby Hledik, PA-C may skip the Virtual Health Assessment.
Existing Clients
Been to Skinlab before? Welcome back!
If it’s been over a year since your last visit, you’ll need to complete a new Virtual Annual Health Assessment before scheduling injectable or laser services. This is a telehealth appointment with our provider to update your health history and medications.
You must complete the Health Assessment before your treatment can be performed.
